This Notice describes how medical information about you may be used and disclosed and how you can obtain access to this information. Please review it carefully.

Who We Are

Le-Vel Rx is a telehealth access and membership platform operated by Le-Vel Rx, LLC.

Clinical services available through the Le-Vel Rx platform are provided by independent licensed healthcare providers participating in the OpenLoop Health provider network and affiliated clinical entities. Medical records and protected health information ("PHI") are maintained by those healthcare providers and clinical entities in accordance with applicable federal and state privacy laws, including the Health Insurance Portability and Accountability Act ("HIPAA").

Le-Vel Rx does not practice medicine, employ healthcare providers, prescribe medications, dispense medications, manufacture medications, compound medications, or maintain patient medical records.

Le-Vel Brands, LLC and its independent Promoters may assist with membership enrollment, customer communications, educational content, wellness-related information, and customer support. However, neither Le-Vel Brands nor its independent Promoters provide medical advice, diagnoses, treatment recommendations, prescribing guidance, or healthcare services.

Your Rights

You have the right to:

  • Get a copy of your paper or electronic medical record.
  • Correct your paper or electronic medical record.
  • Request confidential communications.
  • Ask us to limit the information we share.
  • Get a list of those with whom we've shared your information.
  • Get a copy of this Notice.
  • Choose someone to act for you.
  • File a complaint if you believe your privacy rights have been violated.

Your Choices

You have choices regarding how certain health information is used and disclosed.

You may instruct healthcare providers regarding:

  • Sharing information with family members, close friends, or others involved in your care.
  • Sharing information in a disaster relief situation.
  • Other situations where your authorization or preferences are required by law.

Except as otherwise permitted or required by law, healthcare providers will obtain your authorization before:

  • Using your information for marketing purposes.
  • Selling your information.
  • Disclosing psychotherapy notes except as permitted by law.

If you are unable to communicate your preferences, healthcare providers may share information when they reasonably determine it is in your best interest or when necessary to prevent a serious threat to health or safety.

Our Uses and Disclosures

Healthcare providers and affiliated clinical entities may use and disclose your information without your authorization for purposes of:

  • Treatment.
  • Payment.
  • Healthcare operations.
  • Public health and safety activities.
  • Research where permitted by law.
  • Compliance with legal obligations.
  • Organ and tissue donation activities.
  • Law enforcement and government requests.
  • Judicial and administrative proceedings.
  • Other purposes authorized or required by applicable law.

Healthcare providers may also use information to improve care quality, support healthcare operations, communicate with patients, and administer telehealth services.

Electronic communications may involve the risk of unauthorized access. By providing contact information, you acknowledge and accept these risks to the extent permitted by law.

Healthcare providers and affiliated clinical entities may utilize artificial intelligence and machine learning tools to support administrative functions, documentation, operational efficiency, and clinical workflows. Such tools do not replace the independent medical judgment of licensed healthcare professionals.

How Health Care Providers Typically Use or Share Your Health Information

To Provide Healthcare Services

Healthcare providers may use and share health information to provide, coordinate, and manage your care.

Example: A healthcare provider treating you may consult with another healthcare professional regarding your medical history, treatment plan, diagnosis, or eligibility for a particular therapy.

To Run Healthcare Operations

Healthcare providers may use and share health information to operate their practices, improve patient care, maintain compliance programs, and communicate with patients.

Example: Health information may be reviewed to improve clinical workflows, enhance patient experiences, evaluate treatment outcomes, or support operational and compliance activities.

To Bill for Services

Healthcare providers may use and share health information to bill and obtain payment from health plans, pharmacies, payment processors, or other entities involved in healthcare transactions.

Example: Information about you may be provided to a health plan or other payer so payment can be obtained for healthcare services provided.

Other Uses and Disclosures Permitted by Law

Healthcare providers and affiliated clinical entities may disclose health information as permitted or required by law, including for:

  • Public health and safety activities.
  • Product recalls.
  • Reporting adverse reactions to medications.
  • Reporting abuse, neglect, or domestic violence.
  • Health research.
  • Compliance with federal, state, and local laws.
  • Organ and tissue donation.
  • Medical examiners and funeral directors.
  • Workers' compensation claims.
  • Law enforcement requests.
  • Government oversight activities.
  • National security and other specialized government functions.
  • Court orders, subpoenas, and legal proceedings.

Our Responsibilities

Healthcare providers and affiliated clinical entities are required by law to maintain the privacy and security of protected health information.

We will notify affected individuals if a breach occurs that may compromise the privacy or security of protected health information. Notification will be provided without unreasonable delay and in accordance with applicable law.

Where state privacy laws provide greater protections than HIPAA, those state laws will apply.

We are required to follow the privacy practices described in this Notice and provide individuals with a copy of this Notice.

We will not use or disclose protected health information other than as described in this Notice unless authorized by you or otherwise permitted or required by law.

Changes to This Notice

We may revise this Notice from time to time. Any revisions will apply to information already maintained, as well as information collected after the effective date of the revised Notice.

The most current version of this Notice will be available through the Le-Vel Rx platform.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with the Privacy Officer identified above.

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights at:

No individual will be retaliated against for filing a complaint in good faith.
